Cortana: On the menu I do not have a option to access the notebook?

I only have sign out ,settings and feedback no Home page to access the notebook. How can I fix this why isn't it there?

There's nothing to "fix", all the notebook stuff is gone. MS basically gutted Cortana over the last 12+ months and the goal is to make her a pervasive "assistant" embedded within the OS and apps, vs. as a standalone app.
