Just started getting this code after the GDR3 update.

It occurs with mp3, wav and video files...

At first I thought it was my SD card so I have tried reformatting it with the PC and with my phone with the same results.
I have a Windows 8.1 laptop, all files play fine on it but not on my phone...

Kinda stumped here. Any ideas? I've done a hard reset on my phone and reinstalled all apps. Still get the same result.
The first time I got this error, my phone was not seeing the SD card. It is a Class 10 32GB PNY card.

Any help would be greatly appreciated.

Anyone? I had about 500+ songs and can only listen to about 4 of them....Please advise? Is it my SD card going bad?

I've got the same issue... Lumia 620 with gdr3+black and a brand new sd card. But the sd works for sure.
