Photos app stops responding

I am running Windows 10 Mobile TH2 with my Lumia 630, all is okay till 7th December. From 8th December new images downloaded or received are not shown in photos app, only ghost images are shown with file descriptions but not traceable in file manager app. Even new screenshots are not visible. If i tap on those images message showing "File name changed/ File moved from original location".
Since 8th December Opera mini browser also stopped to refresh its web history & file manager. Whenever i open opera mini it shows history of 7th December, even after clearing of history & cookies result is same.
Also during updating of non system/core apps i am facing error codes like 0x80080204, 0x80073CF9 etc. I soft reset my phone 14 times but all is in vain.

Is there any reason for this anomaly in performance of my Lumia ?

After every soft reset "scanning of sd card for error" is happening. Is it natural process ?

Yes, that happens if you soft reset without ejecting sd card first. Before soft reset I recommend ejecting the sd card first.

Do a repair of the sd card on a Windows PC. It's quicker and more reliable.

Have you checked for any firmware up gradation related to Nokia Lumia?
