just have your nephew buy the phone for you, you can buy the correct sim online for around $3.00 to $ 5.00 and then set it up your self at home I have don this with many Lumia windows phones. hope this helps.
FWIW My 4S came with a T-Mobile SIM in the box. So unless they have different packaging depending on where you buy it, the phone will come with a SIM. Also, I'm pretty sure you can change SIMs without the account owner being involved. T-Mobile has an on-line SIM change facility and I don't think it requires the master account. It would make sense for the master having to approve new lines, but not a new SIM.
