Text to Speech on Lumia 920

Hi
I've setup text to speech on my lumia 920 with ''always on''. 9 out of 10 times it doesn't work after I say ''Read it'' for the message to be read out.
Have I done something incorrectly.
Grateful if someone can advise a solution.
