Why can't my SD cards identify my files?

it keep showing that I've 0 music, 0 photos...+all my files are in other section.
When open music or videos app I can play my content, and pictures lose location and time info.

I've VAIO runs W8.1.2, and my Lumia 720 runs 8.1.1 Black update.

A phone that creates the other file and starts putting media in it needs to be reset to fix the problem.
Start off with a clean, freshly formatted card.

I FIX IT, HOW?
put it on my sister's 625 which runs Cyan update, formatted the SD card, then it works
