Why is my phone making calls when I am already on a call?

Voice Recognition Problem

If I'm taking on my phone to someone and I say - "yes I called Pat, the phone will put whoever I'm talking to on hold and dial my friend Pat. I find Cortana on, even after being shut off. This is becoming very annoying to some of my contact list getting repeat unintentional calls. I'm considering trying to remove Cortana App, but maybe it's something else - HELP PLEASE
