Can I get some help with a few issues after rolling back to 8.1?

Issue after rolling back to 8.1

Since rolling back to 8.1 after the 10149 build of WM10, I have two phone apps. I was quite concerned as default app when pinned to the start screen said "Phone Off" on the tile.

Thankfully when I unpinned it and then pinned the other one to the start screen, it worked normally.

My question is "Does anyone know how to get rid of the extra Phone app - see screenshot.

Hmmm, it looks like you still have the W10M phone app on your phone, it survived somehow and you should take hard reset to get rid off...
