How to disable headphone mode from Lumia 625

I am using my windows phone Nokia lumia 625 from last 2years and no issues observed till now but from last 2 days i am unable to crack the issue found in my phone. It is automatically moved into headphone mode and now i am unable change the mode from headphone due to that i have to attached headphone everytime during my call or viewing videos.
Can anyone help to resolve this issue ?

Clean the lint in the audio port on the phone, with air . Only air.
Do not stick any other thing in the port to try to clean it out. You will bend the contacts and nothing will work

Thanks Moore !! i had tried it multiple time but its not working.. If will play song through headphone for more than 45 mins then after headphone mode disabled but again come back after few mins
