How do I upload my app to the Store?

I have finished editing my app for the Windows Phone Store and I have everything ready such as the .xap file, logo and screeshots. What page do I have to go to, to upload my app. Also is it free to upload?

I'm assuming you made it with visual studio and you have a developer account?

Otherwise, you're going to go to Windows Phone Dev Center if you've purchased a dev account already, or if you need to purchase one.
