If you have a Lumia 930, Lumia 1520, Lumia Icon, or any other Lumia phone and you are unable to train Cortana to use the "Hey Cortana" feature in Windows 10 Build 10586.11, here is what fixed my Lumia Icon.
1) Open Settings> Languages and added English (United Kingdom) and then long press on English (United States) and tap "remove".
2) Tap the "Restart phone" button. Phone will restart.
3) When phone restarts, go back to Settings> Languages and added English (United States) and then long press on English (United Kingdom) and tap "remove".
4) Tap the "Restart phone" button. Phone will restart.
5) When the phone restarts you may not even have to train Cortana, but if you do, you shouldn't have a problem.
I did have a few instances during training where she couldn't hear me, but every 2 or 3 "Hey Cortana's" she did hear me and complete the training and now I can use Hey Cortana in sleep and wake mode.
I hope this helps a lot of people.
