Problem connecting Nokia Lumia 525 to Windows 8 pc

Whenever I try to connect my Nokia Lumia 525 phone to my pc and copy music files and video files from SD Card, sometimes it shows catastrophic failure sometimes does not pastes anything.
please help

it shows message box on my win 8pc when i copy something from my phone's sd card and try to paste it on my pc either it displays Catastrophic Failure or does not carries any action of copy or paste.

Have you scanned you PC for malware / virus ? If so which AV are you using ?

Yes i did but nothing was detected
