SD card errors..Why? and How to prevent?

Have been using WP since 2 years, one issue really annoyed me..."SD Card error" message, phone becomes slower, saving videos+pics takes long time + booting time increases!

Buying a new one seems to be the best solution..not sure about formatting?! , thus starting fresh since all stuff are there on SD.

Now, What causes this? How to prevent them? + i guess soft reset is related to this :/

There are few instances such as pulling the battery out while device in powered, app on sd crashes suddenly and to name a few. Best way is insert that card into PC and use check disk to fix any errors and problem is solved.

What if it shows "error can't be fixed?!?!?"

Did it display by phone or PC? Any Error codes projected by chkdsk for example $MFT cannot be updated because $file1 exists in $USNJournal file etc??

I've seen SD card errors occasionally on my Lumia that were false flags (nothing was wrong, checked on another device). I just ignore the prompt to repair it and it works fine. All my files are intact.
