Volume drops to 0 when headphone are plugged in

Lumia 630 Denim. Phone is over 1 year old, never had this problem before. Did a soft reset twice.

I had this problem with a pair of Monster headphones. It came with two cables and one of them caused exactly what you are describing. If you can, try swapping the cable.
