Why is my Hey Cortana disabled?

Hey Cortana Disabled

I have a Lumia 930, HK CV, with the recent Denim update, which I am using in Canada.

"hey Cortana" is disabled. The message I get is, "Voice Activation: To use Hey Cortana, make sure your region and speech language match your current display language."

I'm not sure exactly which settings to check, but here is what I have:

Keyboard is set to English (United States) - (There is no English (Canada))
Language is set to English (United States) -this shows as Phone Language: enabled; Regional Format: enabled; Speech: enabled; Keyboard: English.
Region is set to Canada. (As I understand Cortana is contextualized for each Region, so I don't want to get United States responses while in Canada.)

Any ideas about how to get Hey Cortana working would be appreciated.

I wanted to send a link but the link does not work at the moment.
But region and language should be UK.
