Is there an app for Windows RT that allows user to sign-in when they arrive?

Looking for Sign-in App

I am looking for an app for Windows RT that allows users to sign-in when they arrive. It's not a business but a group and we keep track of attendance. We use pencil and paper at the moment but it would be nice if we could have them sign-in with the tablet each day and have that export to a word document or PDF for that day.

Touch screen signing would be preferred to check boxes but all suggestions are welcomed

Use one note as that records time when you make a save file and also can have tick boxes too
