weird icon on lock screen.

A few months ago I noticed a square with an x inside and the number one beside it. I opened up texts, nothing, emails, nothing. And now as of today it has the number two beside it. I have checked everything possible, powered off, and even reset my phone and its still there. I took a screenshot for anyone that can identify it!!

A square with an x in the middle is the default app logo for windows apps. My guess is it is either a bug, or something is installed and the developer didn't bother to change the logo.
